Welcome to the Provider page for the CHR Website. Corporate Health Resources is the EAP division of Mesa Mental Health, a managed behavioral healthcare organization located in Albuquerque, New Mexico. CHR and Mesa Mental Health utilize the same provider network and all policies, procedures, and processes are integrated. The Mesa Mental Health Provider Handbook includes a special section for our EAP service providers that contains some valuable information regarding EAP counseling philosophy, claims submission, EAP forms, etc.
